Reciprocal Packet Information

Your application packet must be typed and delivered to the office; items do not need to be sent together.

Required Documents

  • Examination Application found on the Forms page
  • Forward Facing color photograph
  • Fee as marked on application
  • Copy of high school diploma or transcript, or GED certificate
  • FBI Criminal Records Report
  • National Boards sent to they office, if applicable
  • Certified State Verification

All paperwork and fees are due in the Board office by the Exam Deadline date listed on the Calendar.

A passing grade of 75% on each exam is required.

The Kentucky Board of Embalmers and Funeral Directors will provide reasonable accommodation in the administration of all licensure exams for qualified individuals with disabilities. The qualified individual with a disability shall submit, to the Board, documentation from an appropriate professional verifying limitations imposed by his/her disability. Any individuals with disability shall request effective accommodation.
