This Renewal form is for Individuals whom have missed the Renewal period deadline or have been Inactive. Before sending in this form contact the office for your reinstatement amount, also include your $25.00 processing fee. This form must be typed

Level II Apprentice Application
This form is filled out by the supervisor of the apprentice. The apprentice must have served 6 months in the apprenticeship and completed at least 25 removals, 25 funerals and/or 25 embalmings. This form must be typed.

Medical Exemption 2022
For licensed Funeral Directors and/or Embalmers who require a medical exemption for CEU’s but wish to keep their license. This must be filled out with the medical professional and submitted prior to the July Board meeting of the CEU period.

Reciprocal Form
Application for a reciprocal license. Include official state verification, a copy of high school diploma, official copy of college transcripts and National Board scores. Fill out the separate exam form and mail them in together. This form must be typed.

Seventy & Inactive Form
For those license holders who are 70 years or older and not actively involved in embalming or funeral directing and who wish to stop obtaining CE hours per KRS 316.130(3). This form must be typed.
